Normally, alanine aminotransferase is mainly present in tissue cells, with the highest content of hepatocytes, followed by the content of cardiomyocytes, and only a small amount is released into the blood. Therefore, the activity of this enzyme in serum is very low. When the liver, myocardial lesions, cell necrosis or permeability increase, various enzymes in the cells are released, and the activity of the enzyme in the serum is increased. Therefore, the determination of the amount of this enzyme in serum can be used as a basis for diagnosis, differential diagnosis and prognosis.

The increase of alanine aminotransferase is of great significance, and the degree of increase can reflect the degree of hepatocyte damage and necrosis.

Hepatobiliary disease: Acute viral hepatitis, ALT is one of the most sensitive indicators, there will be a significant increase. Chronic hepatitis serum ALT elevation generally does not exceed 3 times the reference value, and sometimes can be reduced to normal. Chronic active hepatitis serum ALT can be increased to more than 3-5 times the reference value. ALT may be slightly elevated in active progressive cirrhosis, but may be normal or slightly elevated during the compensatory period. In patients with primary liver cancer, ALT may increase normally or moderately. When biliary tract diseases such as cholelithiasis cause obstruction, although there is no hepatocyte lesion, ALT may be slightly elevated.
